Location: New York
Job Overview
We are seeking a Nurse Practitioner to deliver high-quality, equitable primary care to Medicare adults in outpatient settings.
Responsibilities- Provide comprehensive primary care, conduct office visits, wellness exams, and health assessments.
- Coordinate care with specialists, testing facilities, and community resources.
- Lead population health initiatives, educate patients, and participate in outreach and home visits.
- Work collaboratively with a multidisciplinary team including physicians, nurses, and support staff.
- Report to the Center Medical Director and support operational goals.
Requirements include a master's or DNP degree, national certification, active licenses, and work authorization. Preferred experience in primary care and population health.
