Inpatient Hospice Clinical Coordinator

A well-established hospice organization organization in New York is seeking a Hospice Inpatient Clinical Coordinator (RN) to support hospice services within inpatient hospital settings. This role partners closely with hospital teams to evaluate patients for hospice eligibility, coordinate General Inpatient (GIP) level of care, and support interdisciplinary hospice services.

The Hospice Inpatient Clinical Coordinator will be responsible for:

• Coordinating hospice referrals, patient evaluations, and admissions in collaboration with hospital partners
• Serving as the RN member of the Hospice Interdisciplinary Team (IDT) for patients receiving General Inpatient (GIP) level of care
• Developing and updating hospice nursing plans of care and coordinating symptom management with inpatient providers
• Supporting transitions of care for patients who no longer meet criteria for inpatient hospice services
• Maintaining accurate, timely, and compliant clinical documentation

Requirements:

• Registered Nurse (RN)
• Graduate of an accredited School of Nursing; BSN preferred
• Minimum of 2+ years of nursing experience in Hospice or ICU settings
• Experience in inpatient or hospital-based environments preferred
