Salary $110,000 - $120,000 per year

Pediatric Home Care RN for Brooklyn

• Provide and coordinate patient care that is cost-efficient and reimbursable under the patient's insurance plan
• Accept case assignments based on client's needs in specific geographic locations
• Conduct initial assessments, case openings, and documentation on the first day of service
• Travel to patients' homes and/or other facilities to deliver direct care
• Complete the Oasis assessment accurately reflecting clinical and functional status of the patient
• Evaluate HHRG and HIPPS scores and match them to service frequencies and durations
• Work with RN supervisor and physician to assign appropriate primary diagnoses
• Participate in weekly PPS meetings to report adherence to service frequencies and durations
• Communicate frequently with PT/OT to ensure adherence to physician-ordered frequencies and durations

• Minimum of one year of experience as a Registered Nurse from an accredited institution
• Prior nursing experience in long-term care or CHHA preferred but not required (will train new graduates)
• Ability to make home visits in potentially unsafe and unsanitary environments
• Access to reliable transportation or ability to use public transportation within Brooklyn, Queens, and Manhattan
