In-person visits: $150 – $210 (average $180).
Orientation hourly rate: $90/Hour.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
This Nurse Practitioner works in collaboration with physicians to provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long-term care facilities. Responsibilities include ensuring development of care planning and access to preventive care services in accordance with member and plan health outcome goals and objectives, acting as a clinical resource to the participating providers and facilities, and serving as a team member and clinical resource to participating providers/facilities.
The NP also serves as a resource and contact for member or designated representative regarding all aspects of member individualized care plan and disease process.
- Provide primary care services to a panel of long-term care facility residents.
- Develop and implement individualized care plans in coordination with physicians.
- Act as clinical resource for participating providers, facilities, and members.
- Coordinate preventive care services and monitor health outcome goals.
- Manage patient disease processes and provide ongoing support to residents.
- Master’s Degree in Nursing, graduate of accredited Nurse Practitioner program required.
- One to two years clinical experience in the role of nurse practitioner required.
- Minimum one year in geriatric specialty.
- Previous clinical RN experience in acute, subacute and/or long-term care setting preferred.
- Prior experience in managed care is a plus.
- NYS RN/NP License, Registration, DEA certificates.
