LPN Nurse Administrator - Parkview Residence

Duties & Responsibilities- The LPN Nurse Administrator is an important leader on the team at their site, responsible for supervision of assigned staff.
- Link clients’ medical and psychosocial needs, making referrals and suggestions for medical treatment, psychiatric rehabilitation, and coordinating information with treating physicians, pharmacies and other treatment collaterals.
- Act as primary medical case manager for assigned clients, managing client health needs by conducting health and vitals checks and coordinating with nursing leadership and other team members as needed to optimize client outcomes.
- Share findings and client needs in treatment team meetings and case conferences and through chart documentation and the maintenance of the onsite medication administration record and controlled substance log, including med counts and follow up to ensure adequate medication supply.
- Work directly with clients to provide health education and to learn and advocate for client health preferences and the development of client health goals through partnership with other team members at the site.
- Provide emergency first aid/CPR assistance when needed.
- Related duties as assigned.
Full time, 37.5 hours per week.
- Monday-Friday 7am-3:30pm
- NYS LPN license required.
- Experience in psychiatric and medical nursing preferred.
- Excellent organizational skills.
- Basic computer literacy required.
- Vaccination preferred but not required.
