Responsibilities
  • Provide comprehensive patient assessments and develop individualized care plans.
  • Participate in continuous professional development and training programs.
  • Administer medications and monitor patient responses to treatments.
  • Set and achieve personal performance targets in patient care.
  • Educate patients and families on health management and disease prevention.
  • Conduct routine health screenings and preventive care services.
  • Collaborate with healthcare teams to ensure coordinated patient care.
  • Utilize evidence-based practices to enhance patient outcomes.
  • Manage patient follow-ups and ensure continuity of care.
  • Document patient interactions and maintain accurate medical records.
  • Engage in community outreach to promote health awareness.
  • Analyze patient data to identify trends and improve care strategies.
Experience/Qualifications
  • Valid Nurse Practitioner license and current CPR certification.
  • Strong commitment to continuous professional development and learning.
  • Proven experience in patient assessment and care plan development.
  • Proficiency in administering medications and monitoring responses.
  • Strong goal-setting skills and ability to achieve performance targets.
  • Ability to effectively educate patients on health management.
  • Strong skills in documenting patient interactions and maintaining records.
  • Must have 1 year of nursing exp
