We are currently seeking multiple Home Health Nurses for Contract-to-Hire positions in the Bronx Area. This is with one of the largest Non-Profit Healthcare Organizations throughout the NYC Area. If you are looking for an opportunity to travel to patients' homes,
Role DescriptionThe Home Health Nurse will provide routine medical care and assistance to patients in their homes, perform wound care, administer medication, monitor overall patient health, and update treatment plans as necessary. Responsibilities also include educating patients and their families about care plans, ensuring comfort, and collaborating with interdisciplinary healthcare teams to deliver personalized and effective care.
Responsibilities- Perform routine home visits to deliver clinical services, including:
- Wound care management and dressing changes
- Medication administration and reconciliation
- General health and functional assessments as directed
- Provide nursing care in accordance with the individualized care plan, physician orders, and best nursing practice standards.
- Assess patient conditions and monitor changes in health status, promptly communicating updates to the care team and physician as needed.
- Collaborate with partner organizations, care coordinators, and internal departments to meet patient needs.
- Educate patients and caregivers on disease management, medication adherence, wound care, and safety in the home environment.
- Ensure timely and accurate documentation using the organization's electronic health record system.
- Adhere to all clinical protocols, regulatory standards, and infection control guidelines.
- Proficiency in core nursing practices and medical knowledge
- Expertise in Home Care, Patient Care, and Wound Care
- Strong interpersonal skills and the ability to communicate effectively with patients, families, and healthcare team members
- Current and valid Registered Nurse (RN) license in the state of New York
- CPR certification and experience in home health or community health nursing is preferable
- Ability to work independently and manage a caseload efficiently
