This is an in-person job located in Developmental Disability Service group homes in Glendale, Queens.
The Licensed Practical Nurse (LPN) will manage the nursing care of individuals, including determining the need for immediate physician attention, executing medical orders from physicians or dentists, and coordinating nursing care with routine facility activities and comprehensive treatment plans. Responsible for overall medical, dental, and nutritional care of all MR/DD residents in the facility. Ensures adherence to applicable regulations and reviews resident progress as appropriate.
Qualifications:
- Licensed by NYS Department of Education
- Experience in community residence and/or nursing homes is a plus
- Respect and consideration for the residents served
Responsibilities:
- Evaluate each client’s need for nursing care periodically and continuously, providing care under the supervision of RN and QIDP.
- Control of communicable diseases and infection through identification, assessment, reporting, and implementation of protective measures.
- Develop written nursing plans for clients requiring care as part of their individual program plan.
- Review each client’s plan quarterly or more frequently for those with acute or chronic conditions.
- Monitor medication effects, ensuring prescribed medications are taken correctly, and dispose of outdated or discontinued drugs properly.
- Ensure clients maintain adequate personal hygiene.
- Coordinate with physicians for illness, injury, and ongoing health services.
- Arrange medical, dental, and hospital appointments.
- Report serious incidents, violations, or rights violations to Program Manager/RN per policies.
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Perform focused interviews to identify patient needs.
- Assess signs of physiological and psychosocial changes.
- Record nursing observations, actions, medications, reactions, vital signs, and needs in medical records.
- Work cooperatively with medical services to ensure treatments are carried out and documented.
- Maintain inventory standards for medicines, supplies, and equipment.
- Account for controlled drugs and narcotics, including inventory management.
- Ensure the safety and security of individuals, assessing their ability to maintain safety and taking necessary precautions.
- Provide information on health care, hygiene, self-treatment, and medication side effects to individuals and families.
- Serve as a liaison or interpreter with outside health providers.
- Train direct support staff in nursing techniques and equipment use.
- Follow all agency policies and guidelines from OPWDD and other funding agencies.
