Nurse Practitioner - Spanish

We are seeking a Nurse Practitioner to provide high-quality, equitable primary care to Medicare-aged adults in outpatient settings.

• Independently conduct office visits, wellness exams, and acute care, while collaborating with a multidisciplinary team.
• Coordinate care with specialists, testing facilities, and support staff to ensure comprehensive patient management.
• Lead population health initiatives and educate patients on their health conditions and care plans.
• Participate in outreach, phone triage, and home visits as needed.

Qualifications include a Master’s or Doctorate in Nursing, national certification in family, adult, or gerontological care, and active licensing. Preferred experience in primary care, understanding of HCC, ICD-10, and passion for team-based, population health models are a plus.
