Wound Care Nurse

Job Overview

Wound Care Nurse at NYC Health + Hospitals. Provide assessment, treatment, and education for patients with wounds, ostomies, continence, and foot wounds. Work under supervision in a multidisciplinary team and ensure compliance with all applicable regulations.

$119,867 per year (fixed).

• Conduct comprehensive and continuous assessment of deep wounds, skin tears, ostomies, severe burns, pressure injuries, and diabetic foot wounds.
• Collaborate with clinical providers to establish individualized, evidence‑based plans of care for complex wounds and continence conditions.
• Provide staff, patient, and family education and counseling regarding wound care, infection prevention, and treatment measures.
• Document wound assessments, treatment recommendations, and progress notes.
• Assist in the development, revision, and implementation of policies, procedures, and guidelines related to wound care.
• Participate in quality improvement initiatives, departmental conferences, and data collection related to hospital‑acquired pressure injuries.
• Perform other professional nursing activities as delegated.

• Valid New York State RN license.
• Certification in one of the following: CWOCN, CWON, CWCN, CWS, or WCC.
• Basic Life Support (BLS) certification.

• Minimum two years of full‑time nursing experience.
• Experience in home health, long‑term care, or hospital settings.
• Knowledge of federal, state, local regulations, and JCAHO standards for home care.
