In-Home Health - Nurse Practitioner or Physician Assistant Queens, NY

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in their homes and helping bridge gaps in care. You will build trusted relationships to make people healthier and bring flexible, comprehensive, and personalized health evaluations to people’s front doors.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You will use an iPad and a clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

• Visit millions of people nationwide to connect them with the right care for their unique needs
• Work with Signify Health as a clinician colleague and fulfill the requirements of the specified role
• Be flexible to travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening)

Primary workplace is the private residence of our members. Environments vary and may present different conditions. Candidates must be able to perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

This pay range represents the base pay for each In-Home Health Evaluation and associated visit; the number of visits per day may vary. Pay is dependent on factors including experience, education, geography, and other criteria. This position may be eligible for additional reimbursement opportunities, including pay per completed Diagnostic and Preventive Services (DPS) as ordered, in addition to base pay. Anticipated weekly hours: 40.

Time Type:
Full time. Pay range: $95,738.00 - $. The actual base salary offer depends on experience, geography, and other factors.
