Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience in various medical settings including hospitals, long-term care facilities, and assisted living environments. As an LPN, you will play a crucial role in providing high-quality nursing care to patients, ensuring their comfort and well-being while adhering to established medical protocols.
Responsibilities- Provide direct patient care under the supervision of registered nurses and physicians.
- Administer medications and treatments as prescribed, including IV infusion and injections.
- Conduct patient assessments and monitor vital signs to evaluate patient health status.
- Assist with diagnostic evaluations and perform basic life support as needed.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families on health management, medication administration, and post-care instructions.
- Support patients with activities of daily living (ADLs) including bathing, grooming, and mobility assistance.
- Participate in discharge planning and case management for ongoing patient care needs.
- Valid LPN license in the state of practice.
- Experience in critical care, emergency medicine, or acute care settings is preferred.
- Familiarity with electronic medical record (EMR) systems such as Epic, Cerner, or eClinical
Works. - Knowledge of ICD-9/ICD-10 coding and medical terminology is advantageous.
- Previous experience working with individuals with developmental disabilities or in memory care settings is a plus.
- Strong communication skills and the ability to work effectively within a team environment.
- Basic knowledge of anatomy, physiology, and infection control practices.
- Experience in pediatrics or geriatrics is desirable; familiarity with hospice or home care is beneficial.
