Nanuet, NY SALARY: $33.00 per hour
HOURS:
Monday - Friday (40 hours a week)

We are seeking a Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in Rockland County
. This role provides essential psychiatric and medical support, health education, and medication management to individuals living with serious mental illness and co-occurring substance use disorders.
Working under the supervision of the ACT Registered Nurse and Psychiatrist/MD
, the LPN will deliver care in participants’ homes and community settings, monitor health, assist with medications, and support the interdisciplinary team in treatment planning and care coordination.
Responsible for on-call shifts monitoring and responding to after-hours calls to the program emergency phone. Shifts are for one week at a time on a rotating basis.
Qualifications- Current NYS LPN license and registration required
- Valid NYS driver’s license required
- Experience in mental health or psychiatric nursing preferred
- Strong communication, organizational, and teamwork skills
- Bilingual (Spanish, Haitian Creole, or Yiddish) a plus
