Position Summary:
We have an exciting opportunity to join our team as a Field Nurse, Home Care - Per Visit
** BROOKLYN Territory** (1 Year of Home Care Experience Highly Preferred!!).

  • Weekday Initial Visit - $91.10
  • Revisits (any day of the week) - $65.00

In this role, the successful candidate provides safe quality therapeutic age appropriate nursing care using evidence based practice. Uses clinical expertise in assessment, clinical decision making, clinical practice and patient education. The nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction.

Job Responsibilities:

  • Performs other duties as assigned.
  • Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age related, environmental, spiritual, transpersonal and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
  • Derives the diagnoses and needs from the assessment data, validates the diagnoses and collaborates with the patient, family, caregivers and physician to develop an individualized plan of care to attain expected quality outcomes.
  • Responsible for the coordination of care by interacting and collaborating with other members of the healthcare team to optimize patient outcomes, patient satisfaction and health care goals.
  • Delivers direct nursing care, ongoing assessment, teaching and training to patients/caregivers that preserves and protects patient autonomy dignity rights values and benefits using evidence based nursing practice.
  • Identifies deficits and barriers to effective knowledge understanding of education provided to patients. Employs strategies to optimize patient engagement. Demonstrates caring behaviors toward patients family and other caregivers.
  • Complete and maintain accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention and evaluation according to Federal Department of Health and The Joint Commission requirements.
  • Assist Agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participate in Agency quality assessment performance improvement initiatives.

Minimum Qualifications: To qualify you must have a Bachelors degree in Nursing (required for employees hired after 9/1/2020; those hired before 9/1/2020 are grandfathered). Current and valid New York State Registered Professional Nurse license. Current and valid BLS certification. Knowledge of standards of practice for all areas of patient care. Excellent communication, oral and written organizational customer service and interpersonal skills.

Basic computer skills.

Required Licenses: Registered Nurse License-NYS

Preferred Qualifications: Bachelors Degree Nursing
