We are looking for a motivated and empathetic Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing clinical team. We specialize in delivering high-quality behavioral health services to individuals residing in nursing homes throughout New York State
. This position is ideal for a practitioner who is enthusiastic about geriatric mental health and dedicated to supporting the well‑being of older adults.

Requirements

Role Overview

  • Perform in-depth psychiatric evaluations and ongoing assessments for residents in skilled nursing facilities.
  • Formulate tailored treatment plans that align with each patient’s diagnosis and clinical needs.
  • Manage psychiatric medications, including initiation, monitoring, and adjustments as appropriate.
  • Partner with physicians, psychologists, social workers, and nursing staff to coordinate comprehensive patient care.
  • Provide therapeutic support, counseling, and guidance to both patients and their families.
  • Educate patients and caregivers on mental health conditions, treatment options, medication use, and coping strategies.
  • Document all clinical encounters accurately and ensure compliance with state and organizational standards.
  • Take part in team discussions, case reviews, and initiatives focused on improving clinical outcomes and care delivery.

Qualifications

  • Active New York Nurse Practitioner license with certification as a PMHNP
    .
  • Ability to manage an independent caseload with professionalism and sound clinical judgment.
  • Strong assessment and diagnostic abilities; solid understanding of psychopharmacology and psychiatric interventions.
  • Outstanding communication skills, with the ability to build trust and collaborate effectively with patients, families, and interdisciplinary teams.
  • Comfortable using electronic health records (EHRs) and general computer applications.
  • A compassionate, patient-first approach to psychiatric care.
