Position Summary

We have an exciting opportunity to join our team as an APP Manager - PA or Certified Midwife. In this role, the successful candidate provides provision, coordination, and assurance of comprehensive health care using advanced practice processes and standards of care, the diagnosis of illness and physical conditions, and performance of diagnostic, therapeutic, corrective and prescriptive measures as appropriate for the patient within a collaborative practice environment.

Supervises and directs a designated group of Nurse Practitioners, Midwives and/or Physician Assistants to optimize service line activities and promote a positive work environment. Serves as a liaison to nursing, care management, hospital administration and other hospital departments. Represents the Office of Advanced Practice Providers to accrediting bodies ensuring institutional compliance.

Job Responsibilities
  • Performs relevant clinical functions based on evidence-based approaches to clinical care, using established guidelines. Performs complete health history, physical examination and psychosocial assessment within the designated practice.
  • Works collaboratively with the practice team, attending physicians, house staff, consulting physicians and other disciplines to identify, develop, implement and modify an appropriate plan of care that considers potential outcomes and goals of care, maximizes patient/family satisfaction, and enhances quality, access, and cost outcomes.
  • Orders laboratory, radiological and diagnostic studies as indicated; differentiates between normal/abnormal findings and follows protocols for abnormal findings, including adjustment in plan of care or discussion with attendings and house staff.
  • Presents and prescribes medications to patients according to NYS law, including controlled substances with DEA authorization. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Evaluates patient responses to treatment and modifies per clinical standards and practice protocols.
  • Coordinates discharge planning with interdisciplinary team, patient and family to assure timely and safe discharge.
  • Acts as a professional role model and promotes a positive work environment, considering age and cultural needs in all patient care activities.
  • Evaluates advanced practice activities that contribute to patient outcomes and uses practice-based evidence to evaluate care and determine changes to improve care.
  • Participates in the development and monitoring of patient outcomes per established practice protocols for quality and performance improvement.
  • Participates in Service Line Quality and Performance Improvement meetings and in Advance Practice meetings and committees.
  • Assists and participates in research protocol activities when applicable and monitors cost-effectiveness related to service efficiency and performance indicators.
  • Collaborates with leadership in recruitment, retention and human resource management of nurse practitioners, midwives and physician assistants; guides and monitors performance and coordination of services to patients.
  • Facilitates onboarding and ongoing professional development of nurse practitioners, midwives and physician assistants; coordinates clinical activities to meet safety and quality measures; provides leadership and team-building.
  • Develops and maintains policies, standards and processes for patient care; sets departmental goals and participates in hospital-wide committees. Serves as liaison between inpatient and outpatient areas to assure continuity of care.
  • Participates in performance appraisals of nurse practitioners, midwives and physician assistants; evaluates fellows, residents and interns as needed.
  • Contributes to nursing/clinical knowledge through writing, research and presentations; educates patients and families to facilitate healthy adjustment to outcomes.
  • Serves as a preceptor to graduate students and mentors staff; facilitates preceptorship for APRN/PA/CM students on applicable clinical services.
  • Develops and revises interdisciplinary and nursing standards and patient/family education materials; collaborates with other departments to ensure seamless transitions of care.
