Job Description
Performs an age‑specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.
Job Responsibility- Assess the patient’s physiologic health status:
- Conduct patient interview, explain policies & procedures, review chart & answer questions courteously.
- Assess gastrointestinal, cardiovascular, respiratory, renal, neurologic status.
- Determine mobility, sensory deficits, prostheses use, skin condition; adjust plan of care as appropriate.
- Assess level of pain and pain management.
- Communicate/document patient physiologic status and plan of care.
- Assess patient psychosocial health status:
- Elicit perception of care and expectations.
- Determine coping mechanism, knowledge level, comprehension ability.
- Identify cultural/ethnic requirements to reduce anxiety & ensure satisfaction.
- Communicate & document psychological status and care plan; support group, counseling services, social services.
- Identify patient outcomes:
- Develop criteria for measurement of outcomes.
- Identify actual/potential patient problems.
- Identify teaching needs based on psychosocial & developmental assessment.
- Develop outcome statement(s) and individualized goals.
- Formulate the plan of care & establish priorities:
- Identify care activities & prioritize for expected outcomes.
- Coordinate cost‑effective use of supplies, equipment & medication.
- Document plan & collaborate with physicians & team.
- Implement the plan of care:
- Maintain vigilance to ensure safety precautions are followed.
- Exercise professional skills related to the plan.
- Reassess patients & revise plan as needed.
- Administer prescribed treatments & use equipment correctly.
- Provide emotional support to patient & significant others.
- Apply scientific principles safely, timely & efficiently.
- Make accurate observations during treatments; report & record as appropriate.
- Keep accurate documentation of treatment, activity, condition & responses.
- Use appropriate documentation methods per policy.
- Act rapidly during any emergency.
- Communicate effectively with family or caretakers.
- Participate in discharge planning & coordinate referrals.
- Provide discharge instructions.
- Evaluate care provided for patient outcomes:
- Measure effectiveness of care.
- Perform variance analysis related to outcome data for improvement.
- Design, implement & evaluate systems to improve unit care.
- Keep accurate documentation.
- Use appropriate documentation methods.
- Collaborate with other care team members:
- Collaborate with physician on patient plan.
- Interpret & implement treatment regimen as prescribed.
- Assist physician during treatments/diagnostics.
- Keep Patient Care Manager or physician abreast of condition/treatment changes.
- Delegate assignments to team members; ensure timely completion.
- Perform grade I‑IV decubitus care:
- Perform preventive skin care measures.
- Apply simple dressings with aseptic technique.
- Apply compresses, irrigations, etc. per procedure.
- Perform other related skin care procedures.
- Administer medications correctly and safely:
- Identify medication actions, dosage, side effects.
- Meet standards on medication administration exam.
- Prepare local solutions & IV drips.
- Administer & document medications correctly.
- Administer IVP medications per policy.
- Educate patients and significant others on drug‑food interactions.
- Participate in patient and family education:
- Explain patient condition.
- Communicate assessment data to team & agencies.
- Utilize teaching opportunities & resources.
- Provide early discharge planning & referrals.
- Evaluate teaching effectiveness via feedback; document.
- Perform related duties, as required.
- Constant standing, walking, stooping, bending, stretching, lifting, pulling, pushing & turning.
- Frequent lifting of objects 10–150 lbs; push up to 200 lbs.
- Occasional sitting; lift with assistance objects >150 lbs; carry up to 50 lbs.
