We are expanding our behavioral health services and are seeking a licensed Psychiatric Nurse Practitioner to provide ongoing psychiatric care across affiliated post-acute and residential facilities. • Conduct initial psychiatric evaluations and manage longitudinal follow-up care
• Provide medication management and monitor treatment effectiveness
• Develop individualized care strategies for patients with chronic psychiatric conditions
• Coordinate closely with physicians, nursing staff, social workers, and facility leadership
• Educate patients and families on diagnoses, medications, and treatment plans
• Deliver evidence-based, patient-centered psychiatric care
