Responsibilities
- Clinically oversee a cluster of nurses in the Brooklyn DDSO, managing multiple sites and functioning as an RN1 as needed.
- Ensure nursing case management across multiple locations, with caseloads based on medical acuity.
- Oversee and supervise RNs and LPNs who in turn supervise AMAP staff providing Direct Support Assistant health care under the Nurse Act Exemption.
- Ensure all medical follow‑up is completed in a timely manner.
- Provide in‑service training on nursing plans and other staff development activities, including medication administration.
- Engage in orientation and mentoring of new RNs and LPNs.
- Complete staff evaluations and monitor/complete nursing audits for the cluster.
- Collaborate with the Nursing Supervisor / Director of Nursing and team to review audit findings and develop corrective action plans.
- Monitor individuals’ medical and health needs and services.
- Interact with primary medical specialists, hospitals, and various clinical staff as necessary.
- Advocate for individuals’ health‑care needs and maintain ongoing communication with medical liaison/designee.
- Provide support to site staff in maintaining health services for individuals.
- Utilize appropriate reporting mechanisms and chain of command to address individual or staff issues.
- Respond to emergency requests as promptly as possible, both within and outside of caseloads.
- Complete nursing assessments and related documentation via electronic health record systems (e.g., RN Notes, nursing logbook) thoroughly and within established time frames.
- Provide individual‑specific nursing interventions and answer staff questions regarding medical or nursing issues.
- Maintain current PONS for each caseload, ensure all PONS are signed and stored in the EHR.
- Deliver health‑related training and consultation to residential staff.
- Annually conduct a review of the medical section of Life Plans/CFA and a medication audit for each home/day program.
- Participate in the nurse on‑call rotation.
- Obtain individuals’ informed consent for medications in accordance with BDDSO policy.
- Complete required medication error documentation and staff medication error remediation.
- Maintain professional skills by attending Nursing Discipline meetings, completing mandatory DDSO and discipline‑specific training, identifying improvement opportunities, and completing special projects or assignments.
- Maintain computer proficiency with Microsoft Word, Excel, and Electronic Health Record software.
- License and current registration as a registered professional nurse in New York State.
- At least two years of post‑licensure clinical nursing experience, with one year of direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
- A bachelor’s degree in nursing may substitute for one year of general post‑licensure clinical nursing experience.
- A master’s degree in nursing may substitute for two years of general post‑licensure clinical nursing experience.

- Posted Hiring Salary includes statutory hiring rate for PS&T Grade 20 ($74,193), increase hiring rate ($9,980), geographic pay differential ($35,000), and downstate adjustment ($4,000). The total job rate is $94,121.
