We are seeking a friendly and compassionate Travel Nurse Practitioner/Physician Assistant with experience in Cardiology to join our team and support patients in the Patient Health Management (PHM) program. In this role, you will work closely with care coordinators across various providers and settings to promote improved health outcomes. You will facilitate patient assessments, assist with communication between facilities and patients, and ensure accurate documentation through our proprietary software system.

You will receive regular support and guidance under the supervision of an off-site Collaborative Physician. This is an exciting opportunity to make a meaningful impact on patient health outcomes while working in a dynamic and supportive team environment.

• Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with cardiac conditions and report through our proprietary software system
• You will be cross-trained in other areas of care management
• Work under the supervision of an off-site Collaborative Physician
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes

• DEA License
• 6 months of experience in long-term care
• Reliable transportation
• Must be willing to travel to different nursing homes within the state of New York (100% commitment)
