Job Summary:
The RN Clinical Care Coordinator promotes coordination of care to our residents by monitoring, documenting, assessing, and evaluating interdisciplinary care from admission to discharge. They are also responsible for assisting in overseeing the daily functions of the unit in conjunction with the Unit Manager.
Duties/Responsibilities:- Reviews 24-hour report. Makes rounds and reviews everyone on report related to any clinical problems or issues. Documents any findings in the medical record.
- Confers with Unit Manager, Nurse Practitioner/MD, resident, resident family and staff as indicated throughout the shift.
- Reviews any previous admissions to ensure all skilled nursing documentation is complete. Makes certain that all nursing orders for care are complete and indicated on the resident care profile. Makes certain that the LPNs and CNAs are aware of any changes needed on the delivery of care.
- Assists the daily operations of the LPNs and CNAs with the Unit manager.
- Follows-up on any residents who are demonstrating new clinical issues and confers with Unit Manager as indicated.
- Oversees the admission process for each resident. Responsible for the completion of the entire nursing assessment phase of the admission. Works in the conjunction with the LPNs to make certain that admission NP/MD orders are written, transcribed, and sent (faxed) to the pharmacy.
- Monitors delivery of care by completing resident rounds; performs thorough skilled nursing documentation to support services provided for all payers; identifying progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with residents in conjunction with the direct care providers; interacting with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults;
reporting personnel and performance issues to the unit manager; maintaining ongoing communication with utilization review staff regarding variances from the care plan or transfer/discharge plan. - Assists in the educational process of skilled nursing documentation in conjunction with Staff Development Coordinator as necessary
- Orients and educates residents and their families by explaining what to expect during their stay, initiating the interim care plan, and providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.
- Evaluates outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team; implementing team recommendations.
- Oversees discharge planning process to ensure a smooth transition back to the community.
- Updates resident care plans as clinical changes develop.
- Assists in Unit Manager duties when Unit Manager is absent.
- Oversees the renewal process, including faxing the pharmacy and checking for changes.
- Performs weekly “skin-rounds” with Nurse Manager to follow-up on skin with Charge Nurses.
- Attends family and/or Medicare meetings as needed.
- Assists with supervising facility on weekend shifts as needed.
- Perform other duties as assigned.
- Demonstrated organizational and leadership skills.
- Knowledge of the process of aging as it relates to health care, resident rights and abuse reporting regulations.
- Knowledge basic of principles of infection control, resident safety, and standard precautions.
- Must be currently licensed in New York State as a Registered Professional Nurse.
