Psychiatric Mental Health Nurse Practitioner (PMHNP) – Queens, NY

Our team is looking for Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing behavioral health network in Queens, NY .
In this role, you will deliver compassionate, high-quality psychiatric care to residents in long-term care and rehabilitation facilities. This is a meaningful opportunity to support the mental and emotional well-being of older adults while enjoying a collaborative and supportive work culture.

Requirements

Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments.

Develop and adjust personalized treatment plans that meet each patient’s unique mental health needs.

Prescribe, monitor, and manage psychotropic medications, ensuring safety and therapeutic effectiveness.

Work collaboratively with an interdisciplinary care team —including physicians, nurses, psychologists, and social workers—to coordinate holistic care.

Provide therapeutic guidance, counseling, and education for patients and families to support treatment adherence and understanding.

Maintain accurate and compliant documentation in the electronic health record (EHR) system.

Participate in team meetings, case conferences, and quality improvement initiatives to promote best clinical practices.

Qualifications

Active New York State Nurse Practitioner license with Psychiatric-Mental Health certification (PMHNP) .

In-depth knowledge of psychopharmacology and evidence-based treatment for geriatric populations.

Excellent assessment, communication, and collaboration skills.

Comfortable working both independently and as part of a team-based care model.

Proficiency in EHR systems and general digital literacy.
