Purpose:
We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Gravesend/Flatlands, NY region. You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.
- Provide high-quality primary and palliative care to patients in and around Gravesend/Flatlands, NY
- Assess health by performing physical examinations, obtaining and updating medical histories, and creating care plans
- Communicate effectively with office staff and clinical operations across internal platforms
- Deliver patient-centered care to a panel of patients
- Attend quarterly medical staff meetings
- Develop impressions, diagnoses, and treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and appropriate medically necessary interventions
- Board Certified with a valid NY NP or PA certification
- Strong clinical skills to accurately assess, diagnose, and treat patients
- Positive demeanor and strong people skills
- Ability to problem solve and work independently
- 1–2 years of primary care experience with geriatric patients; hospice experience preferred but not required

- 105,000 – 160,000+ USD, depending on experience and incentives
