Job specification

• Conduct comprehensive patient assessments, collect health histories, monitor vital signs, and identify care priorities.

• Collaborate with physicians, advanced practice providers, and allied health professionals to coordinate patient care plans and referrals.

• Provide education and counseling on lifestyle, nutrition, immunizations, and preventive screenings.

• Perform routine clinical tasks such as wound care, immunizations, injections, specimen collection, and point-of-care testing per state nursing regulations.

• Maintain accurate, timely electronic health records (EHR) compliant with HIPAA and clinic policies

Candidate requirements

• Valid RN license in New York.

• Minimum of 1 year of long-term care experience required.

• Strong leadership, problem-solving, and communication skills.
