Position Summary

The primary role of the Nurse Practitioner (NP) is to work collaboratively with the Advanced Cardiac Care and Transplantation team. Begun in 1984, our transplant program has performed transplant in over 650 patients and currently averages approximately 20+ transplants annually. In addition, we have an established MCS program offering the full complement of temporary and durable devices to approximately 15-20 patients per year, including bridge‑to‑transplant in complex CHD patients and destination VAD therapy.

The NP position will include clinical responsibilities for inpatient and outpatient care before and after transplantation including participation in the evaluation/listing process. Additional responsibilities will include clinical care of patients in our cardiomyopathy, cardio‑oncology, advanced Fontan care, hypertrophic cardiomyopathy, and cardio‑genetics practices. There are a multitude of QAPI and education opportunities within the program as well.

A successful candidate will be expected to work collaboratively with Physicians, NP peers, Registered Nurses, Research Coordinators, other members of the multidisciplinary care team, and administrative support staff. They will be engaged in all aspects of patients' clinical care and work with other members of the care team to provide seamless care across all venues including inpatient and outpatient. They will be expected to render direct patient care including but not limited to obtaining patients' history, performing comprehensive and targeted physical examinations, ordering and evaluating diagnostic tests, and diagnosing/managing chronic as well as acute care problems.

A large component of the role revolves around Counseling and educating patients and their caregivers on disease processes and care management. The NP will be engaged in all aspects of patients' clinical care and work with other members of the care team to provide seamless care across all venues including outpatient, inpatient and clinical research venues. The NP is involved in indirect patient activities with the goal of improving the quality of care.

The NP is expected to participate in team meetings and monthly multidisciplinary meetings.

Responsibilities
  • Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements.
  • Perform and document comprehensive surgical, medical and psychosocial health histories, examinations (document pertinent positive and negative findings), and initiate/maintain a problem/medication list.
  • Order and interpret diagnostic studies including but not limited to laboratory and imaging studies, diagnostic impression, and/or form a differential based on the subjective and objective data obtained.
