Become part of the HOPE At Home family and apply today! We are currently hiring an experienced Home Health Care RN, preferably with Mediport, PICC Line, Wound Care, and Infusion experience.
The role involves providing skilled nursing care in patients' homes throughout the Tri-County area (Oakland, Macomb, Wayne, Livingston, Washtenaw). The RN will work under physician orders and the supervision of the Clinical Manager/Nursing Supervisor.
Qualifications- Licensed RN in Michigan
- One year of nursing experience
- Kinnser EMR experience preferred
- Home Healthcare experience preferred
- Experience with Wound Vac, PICC line, Mediport preferred
- Pediatric experience desirable
- Provide services as per the care plan
- Conduct initial and ongoing patient evaluations
- Initiate and revise care plans
- Perform specialized nursing procedures
- Document patient visits and care summaries
- Coordinate with other healthcare providers
- Inform team of patient condition changes
- Counsel patients and families
- Participate in training and in-service programs
- Adhere to agency policies
- Process orders, notify physicians, and complete documentation
- Refer patients to specialists as needed
- Supervise and teach nursing staff
- Conduct patient care conferences
- Participate in quality improvement and review processes
- Provide total patient care when necessary
- Take on-call duties, including nights, weekends, and holidays
- Complete required assessments and documentation timely
Indoor office work, home visits, and travel within service area.
