LPN - Licensed Practical Nurse Team Leader

Join Elderwood at Lockport’s New Era of Care. This is a full‑ or part‑time position with evening shifts available. You will work under a highly respected Director of Nursing in a supportive environment that values team collaboration and staff success.
SalaryStarting at $28 per hour. Pay rates begin at $28/hour and may increase based on experience. Additional earning opportunities include +11% with the Pay‑in‑Lieu‑of‑Benefits option, shift differentials, and a $5,000 sign‑on bonus for some locations.
Shift Options- Evenings: 3:00 PM – 11:00 PM (every other weekend required)

- Lead and coordinate daily clinical care for residents and sub‑acute patients
- Plan, implement, and evaluate care to ensure highest quality and compliance standards
- Work closely with interdisciplinary teams to achieve best outcomes for residents
- Build relationships with residents and families in a supportive, home‑like environment
- Administer medications and treatments to assigned residents as ordered by physicians.
- Direct and monitor the personal care duties and nursing care procedures performed by Nursing Assistants.
- Assist Nursing Assistants with direct care of residents as needed.
- Report to Unit Manager or Charge Nurse Supervisor any unusual symptoms or changes in condition.
- Follow the Plan of Care for each resident and monitor assistant performance in its implementation.
- Manage controlled substances, ensuring safe storage and distribution.
- Document care in nursing notes, medical administration records, treatment records, and other required records.
- LPN or RN license in the state, current and active.
- Experience in a geriatric setting preferred.
- Ability to supervise and work well with other personnel.
- Strong communication skills, both verbal and written.
- Respectful, caring attitude toward residents.
- Physical stamina for constant activity.
- Ability to perform tasks to established standards of excellence.
- Proficient computer skills.
- Intermediate or advanced proficiency in English.
