About the role

This pilot program is a Cash Plus model that combines unconditional direct cash transfers (DCTs) with optional supportive programming and services for 9 to 12 months to support young adults ages 18 to 24 to secure safe and stable housing. The Cash Plus approach recognizes that young people often face both financial and non‑financial constraints to long‑term housing stability and thriving.

The cash amounts will be delivered over a 9 month period and supportive programming will be delivered over the time frame with an additional 3 months of support provided after the cash transfers end.

What you’ll do

The DCT Care Coordinator will serve as the central point person and connector of service support for youth participating in the “Cash Plus” program. Serving as the key connector of optional support services, they work in conjunction with multiple members of the CHNY programs team as well as community agencies and supports that are involved with the youth. The Care Coordinator will enroll youth in the DCT program, provide care coordination services, support supplemental programming, and collaborate with established programming and/or outside partners.

The DCT Care Coordinators are the primary point of contact for youth in this program and act as a resource and support for youth as needed. The DCT Care Coordinators will work closely with the youth and other service providers working with the youth to either identify goals, or coordinate/connect the youth to resources to meet their identified goals.

Essential Job Functions
  • Enroll and track participation of the youth involved in the pilot program.
  • Submit and manage documentation for youth incentives and payments.
  • Assist in the design of promotional materials (monthly calendar, fliers, brochures, etc.) to inform and support participant engagement in the program.
  • Follow‑up with CHNY wrap‑around services by communicating with identified support staff.
  • Administer periodic youth advancement workshops on housing sustainability, money management, and community resources in order to empower participants to sustain stability.
  • Effectively communicate with youth and CHNY staff.
  • Coordinate with CHNY Cov Works department, CHNY Case Managers, etc. for necessary training for the participants in the program.
  • Identify relevant housing resources for program participants and support young people in accessing those resources.
  • Aid youth with housing opportunities by connecting youth to necessary staff and support.
  • Attend in‑service training as required; participate in staff meetings, planning meetings and other meetings as needed.
  • Enter data appropriately into online database ETO as assigned by supervisor in support of the data integrity plan at CHNY.
