Care Manager Assistant - Spanish Speaking (Manhattan, NY) (Education)

Job Requisition

The Care Manager Assistant provides face-to-face visits, ongoing support, and care coordination to members. Responsibilities include face-to-face visits, communication and follow‑up with members, reviewing the person‑centered service plan with members/families, and coordinating covered and non‑covered benefits. Fieldwork is required.

• Conduct multiple types of mandatory face‑to‑face visits based on need
• Schedule members due for face‑to‑face visits
• Meet weekly productivity expectations
• Complete necessary documentation, scripts, activities, and follow‑ups ensuring all items are addressed at the time of visit
• Understand and support company‑wide goals based on direction
• Participate in weekly team meetings and ongoing training
• Other duties as identified as necessary part of employee's role

• Must be able to travel and conduct home visits
• Knowledge of member assessment procedures
• Excellent interpersonal skills; ability to speak effectively with members and agency employees
• Demonstrate effective communication with members of all ages and socioeconomic levels
• Ability to read, interpret documents, understand, and execute workflows
• Appropriate background and experience in Care Management and/or MLTC services
• Bilingual preferred (Cantonese/Mandarin) or (Spanish)

Pay: $29‑$31 per hour

On the road - Manhattan and The Bronx

Schedule:

Monday to Friday
