Caregiver and Family Advocacy and Support Services (CFASS) Representative

Location:

New York City, NY (Immediate openings in Far Rockaway, Bronx, and Staten Island)
About the Role

We are seeking a compassionate and dedicated Caregiver and Family Advocacy and Support Services (CFASS) Representative to join our team supporting the Home and Community-Based Services (HCBS) program in New York City. The CFASS Representative will serve as a vital resource for caregivers and families, providing advocacy, education, and support to ensure access to quality care and services.

Key Responsibilities
  • Provide direct support and advocacy for caregivers and families participating in the NYC HCBS program.
  • Assist families in navigating healthcare, social services, and community resources.
  • Educate caregivers on available benefits, rights, and responsibilities under the HCBS program.
  • Collaborate with healthcare providers, social workers, and community organizations to coordinate care and services.
  • Identify and address barriers to care and service delivery for families and caregivers.
  • Document interactions, maintain accurate records, and report on service outcomes.
  • Participate in outreach and engagement activities to raise awareness of CFASS services within the community.
Qualifications
  • Experience working with caregivers, families, or individuals receiving home and community-based services.
  • Strong knowledge of HCBS programs, caregiver support services, and community resources in NYC.
  • Excellent communication, interpersonal, and advocacy skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in documentation and case management practices.
  • Familiarity with cultural competency and sensitivity in diverse communities.
  • High school diploma or equivalent required; relevant certifications or higher education preferred.
