Peer Specialist - Clinical Rapid Response

Base pay range: $16.74/hr – $21.60/hr
Reporting to the Program Coordinator, the Peer Specialist for the Clinical Rapid Response Team (CRRT) will provide immediate support and assistance to individuals experiencing health crises, working to prevent future crises by leveraging lived experience to build trust, empathy, and connection. The role is integral to an interdisciplinary team, supporting clients through the crisis intervention process and fostering a culture of person‑centered, strength‑based care.
ResponsibilitiesProgram / Facilities
- Provide support and resources/referrals for individuals to meet health objectives, including health, mental health treatment, substance use services, non‑medical wellness services (yoga, meditation, exercise), and social/community integration activities.
- Provide guidance, support, and side‑by‑side skill modeling on an ongoing basis.
- Respond to crises throughout the agency to help avert hospitalizations; when necessary, ensure admission and follow‑up care before discharge.
- Create preventative and wellness plans that may include goals, objectives, and interventions.
- Promote client self‑advocacy skills.
- Organize and facilitate individual and group wellness activities, workshops, groups, and social activities for high‑acuity clients.
- Complete incident reports and distribute related notices within 24 hours.
- Maintain and submit all necessary documentation and reports accurately and in a timely manner, in compliance with Urban Pathways, regulatory and funding agency policies.
- Enter progress notes after meetings with individuals within 48 hours.
- Update information in electronic health records as required.
- Document incidents and distribute related notices within 24 hours when witnessed.
- Submit all required paperwork for supervisor approval within 48 hours; file paperwork in electronic charts as required.
- Develop and maintain relationships with community services and resources, emphasizing substance/alcohol use programs, health, mental health programs, and benefits.
- Attend and participate in external meetings with stakeholders.
- Communicate verbally and in writing in a professional manner.
- Attend staff meetings, trainings, and supervisory sessions as required.
- Respond to all communications, including telephone, email, and other inquiries in a timely manner.
Performs all other duties as assigned and required.
QualificationsEducation and Experience
- High School diploma or GED required.
- Completion of Peer Advocacy/Specialist Training Program required.
- Experience using behavioral health services (mental health, co‑occurring substance use treatment) as a client.
- Ability and willingness to work a flexible schedule including evenings.
- Experience with individuals with a history of homelessness, mental illness, substance use, and chronic medical conditions (preferred).
- Good computer skills and ability to learn programs such as Microsoft Office and data programs (e.g., AWARDS).
- Good written and verbal communication skills.
- Good interpersonal skills and ability to work as part of a team; ability to speak foreign languages is a plus.
- Required to work a flexible schedule including evenings.
- Valid driver’s license with clean driving record (plus).
- Ability to do field work, program visits, client escorts, and use public transportation.
- Ability to sit, stand, walk, bend, stoop, push, pull, lift (up to 25 lbs.), type, write, read, speak, see, and hear.
