CARE NAVIGATOR – Family Health Center of Harlem
Brief Description
Works as part of an interdisciplinary team to assist patients address personal and environmental difficulties that could negatively affect treatment adherence. Care Navigators ensure the execution of service plans by accompanying patients to medical appointments, promoting healthy living, providing social support, health education literature, conducting home visits, community outreach, and obtaining health home consents. Care Navigator adheres to evidence‑based guidelines and carries out pre‑visit planning, providing case management to address each patient’s individual needs in relation to treatment adherence—including but not limited to transportation to medical appointments and childcare.
Requirements
- HS Diploma or GED required.
- Knowledge of programs and services.
- Ability to write professional reports and correspondence.
- Ability to work independently and as part of a team.
- Knowledge of health care.
- Ability to identify resources and utilize problem‑solving skills to meet patients’ individual needs.
- Excellent interpersonal skills.
- Excellent time‑management and organizational skills.
- Excellent computer skills required.
- Bi‑lingual, Spanish preferred.
