The Judge Group is seeking an experienced Nurse Practitioner to perform in-person, annual preventative health risk assessments for Medicare, Medicaid, and ACA members. Assessments are conducted in patients' homes in various counties throughout New York. This role is ideal for compassionate and motivated providers comfortable in residential environments and committed to high-quality care.
Work in an available county.
Broome, Chautauqua, Livingston, Monroe, Oneida, Onondaga, Ontario, or Wayne
Schedule & Compensation- Hours: day and evening appointments available (60–75 minutes per visit)
- Monthly Commitment: at least 15 visits per month
- Compensation: competitive rate per completed assessment
- Mileage Reimbursement: after first 30 miles/day at IRS standard rate
- Conduct comprehensive risk assessments based on health plan standards
- Document clinical findings and submit notes within 24 hours
- Respond to charting queries within 48 hours
- Knowledge of CMS and ACA risk adjustment models
- Master’s or Doctorate degree from accredited program (FNP, AGNP, ANP)
- Home health experience required within the last 5 years
- Current NP license in New York
- Board Certification (ANCC, AANP, or NCCPA)
- Current Medicaid Medicare PTAN
- Active BLS Certification
- Valid driver’s license and proof of auto insurance
