Job Overview – Field Outreach Coordinator (Home Care):

Compensation: $40,000/year

Location:
Bronx, NY

Schedule:
Monday to Friday, 9:00 AM – 5:00 PM

Our client is hiring a Field Outreach Coordinator (Home Care) for a community-based role in Bronx, NY. This entry-level position involves in-person outreach, patient support, and relationship-building to promote home care services and assist with the intake process. Bilingual fluency in Spanish and English is required for success in this field-based role.

Responsibilities as the Field Outreach Coordinator:

• Community Engagement: Attend events and visit senior centers to promote services and distribute marketing materials.
• Patient Support: Assist with NYIA evaluations, deliver documents, and collect intake forms in patients’ homes.
• Referral & Relationship Building: Maintain strong ties with community partners and referral sources.
• Field Reporting: Share real-time updates and feedback with intake and marketing teams.
• Enrollment Assistance: Collect insurance info, physician confirmations, and signatures to support the intake process.

Qualifications for the Field Outreach Coordinator:

• Language Requirement: Bilingual in Spanish and English is required.
• Education: High school diploma or equivalent is required.
• Experience: 1+ year of experience in healthcare, home care, community outreach, or a similar field-facing role.
• Technical Skills: Comfortable using smartphones, email, and basic applications for communication and documentation.
• Skills & Attributes: Excellent interpersonal, organizational, and customer service skills with the ability to work independently and manage time effectively.
• Mobility: Must be comfortable traveling throughout the Bronx for in-home visits and field activities.
