Location: Manhattan, In-Office & Field-Based

Role Overview

We are seeking a dedicated clinical professional to support direct client care while providing leadership within a multidisciplinary Assertive Community Treatment (ACT) team. This role combines hands‑on clinical work, staff supervision, and program support in a fast‑paced, community‑based mental health setting.

Key Responsibilities Clinical & Direct Services
  • Manage a small caseload, completing client assessments, treatment planning, and ongoing care coordination
  • Connect clients to psychiatric care, medication management, entitlement benefits, and community-based services
  • Provide outreach and client support in residential facilities, hospitals, and community settings
  • Maintain accurate and timely documentation in compliance with program, agency, and OMH requirements
  • Participate in an on‑call rotation and provide coverage as needed
  • Provide clinical supervision to staff and interns, including case review and skill development
  • Support recruitment, onboarding, and training compliance for ACT team staff
  • Monitor staff documentation and data reporting in required systems (AWARDS, NYCMED, CAIRS)
  • Assist with program operations, scheduling, and policy implementation
  • Assume Program Director responsibilities as needed
  • Work collaboratively as a member of a multidisciplinary ACT team
  • Participate in team meetings, case conferences, supervision, and professional trainings
Requirements:
  • LMSW (Licensed Master of Social Work)
  • Strong verbal and written communication skills
  • Excellent organizational skills with strong attention to detail
  • Proficiency in Microsoft Office, particularly Word and Outlook
  • Ability to use or quickly learn electronic health record systems and databases, including AWARDS, CAIRS, NYCMED
    , and other funder‑required software
Preferred Experience
  • Prior ACT Team experience
Physical & Travel Requirements
  • Ability to walk, climb stairs, and travel throughout the community
  • Comfortable using public transportation as required
