Monday to Friday, 8:00 AM to 5:00 PM
Our client is hiring a Care Manager in Brooklyn, NY, to support children and youth with complex medical, behavioral, and developmental needs. This field-based role involves conducting assessments, developing care plans, and coordinating services across providers, families, and community resources. Bilingual Spanish/English proficiency is strongly preferred, and familiarity with the CNNX system is a plus.
Responsibilities- Enrollment & Documentation: Support families through enrollment and complete required forms, including consents, demographics, and eligibility verification.
- Assessment & Care Planning: Conduct CANS-NY assessments and health screenings, and develop individualized service plans based on child and family needs.
- Care Coordination: Collaborate with families, case planners, and providers to align services across medical, behavioral, and social systems.
- Ongoing Monitoring & Transitions: Maintain regular contact with clients, monitor changes in condition, and manage transitions between care settings.
- Compliance & Follow-Up: Ensure timely documentation, billing, and follow-up care using systems like CNNX, while supporting treatment adherence.
- Education: Bachelor’s degree in Health Services, Social Work, Psychology, or a related human services field is required.
- Experience: 1+ years of experience working with children and youth, particularly those with mental health, developmental, or physical disabilities.

Familiarity with the New York State CNNX (Connections) system is preferred, with the ability to learn and use care management platforms upon training. - Skills & Attributes: Excellent communication, time management, and documentation skills, with a collaborative and client-centered approach.
- Licensure: Must possess a valid New York State .
