Physical Therapist

Community Care Physicians is seeking a motivated Physical Therapist to join their full‑time outpatient practice located in our state‑of‑the‑art building, Wellness Way.

Eligibility & Compensation:

• This position is eligible for a $5,000.00 recruitment/retention bonus.
• Annual salary: $78,000.00 – $. Compensation may include overtime pay and other benefits in addition to base salary.

As a Physical Therapist, you will provide direct patient care, managing patient/client assessment, examination, diagnosis, prognosis, and interventions aimed at maximizing functional outcomes. You will document treatments and progress per professional, departmental, and regulatory policies.

• Formulate comprehensive treatment programs by evaluating patient history, conditions, impairments, disabilities, and functional status.
• Prioritize patient care needs.
• Assess patient needs throughout the continuum of care and provide timely disposition, equipment, education, and treatment recommendations.
• Complete all documentation in accordance with professional, organizational, regulatory, and facility compliance standards.
• Ensure all clinical and payer‑required documentation is concise, pertinent, legible, and compliant with organizational requirements.

• Graduate of an approved Physical Therapy program (Bachelor of Science, Master’s, or Clinical Doctorate).
• Current New York State Physical Therapy license and registration.
• One year of physical therapy experience preferred; new graduates welcome.
• Strong verbal and written communication skills.
