Woodhaven Adult Home, located just 3 miles south of Port Jefferson's waterfront village, is seeking a compassionate and skilled Home Health Aide to join our team! As a Home Health Aide at Woodhaven, you will have the opportunity to make a meaningful difference in the lives of our residents by providing individualized care and support in their homes.

• Assist residents with daily living activities, such as bathing, dressing, and medication management
• Provide emotional support and companionship to residents
• Assist with light housekeeping and laundry duties
• Monitor and report any changes in resident condition or behavior
• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Work in a fast-paced, dynamic environment with a focus on resident-centered care
• Enjoy the satisfaction of knowing you are making a difference in the lives of our residents
• Participate in ongoing training and education to enhance your skills and knowledge

• Must be a New York State Certified Home Health Aide (HHA)
• Recent Physical, including a drug screen, PPD, and Rubella and Rubeola Titers
• Ability to arrive timely with a self-driven and positive attitude
• Ability to interact with the Residents with respect and dignity
