Location: New York

Under general supervision, the Case Worker provides non-clinical social services and case management support to clients and patients, including recipients of public assistance, food stamps, and medical assistance. The role supports adults receiving or requiring institutional care or protective services through assessment, service planning, referral coordination, and ongoing care management.

Duties & Responsibilities
  • Provide social services and case management to individuals and families with complex needs
  • Conduct intake, assessments, and eligibility determinations for social and medical assistance programs
  • Identify, develop, implement, and monitor individualized social service plans
  • Coordinate and manage nursing home placements and institutional care transitions
  • Send, track, and follow up on referrals to healthcare providers, facilities, and community resources
  • Support adults receiving or requiring institutional care or protective services
  • Perform counseling and investigatory activities related to service needs and eligibility
  • Maintain accurate, timely, and compliant case documentation
  • Collaborate with interdisciplinary healthcare teams, service providers, and families
  • Participate in case reviews, meetings, and departmental training
Required Qualifications (MUST HAVE)
  • Bachelor’s degree from an accredited college or university
  • Minimum 3 years of experience in case management or social services
  • Direct experience with nursing home placement or institutional care coordination
  • Experience sending, tracking, and managing referrals
  • Experience working with vulnerable populations receiving public or medical assistance
  • Strong documentation, communication, and organizational skills
Preferred Qualifications
  • Experience in hospital, managed care, or long-term care settings
  • Familiarity with EPIC and Care Port systems
  • Knowledge of Medicaid, Medicare, and public assistance programs
