Scope of Position

Seeking a full-time Psychiatric Nurse Practitioner to split time between two Safe Havens located in the Bronx and Harlem, NYC. Under the supervision of the Program Director, the Psychiatric Nurse Practitioner will work within a 42-bed Safe Haven serving chronically homeless individuals experiencing mental health and/or substance use conditions. Responsibilities include providing psychiatric and health assessments, conducting medication evaluations and monitoring, documenting services in medical records, and collaborating with the treatment team in the care of clients.
One of the Safe Haven sites, Old Broadway, is located on 125th Street in Harlem, Manhattan.
Safe Havens operate 24/7, 365 days a year. Flexibility is provided regarding days and times for scheduling. This is a full-time, in-person position. Salary and benefits are competitive. Malpractice insurance is covered by the agency. The Bridge is an HRSA National Health Service Corps loan repayment site.
Essential Position Functions- Provide psychiatric evaluations for clients as a requirement of the clients’ housing applications.
- Provide health assessments, as needed, consistent with education and training.
- Perform medication evaluations, education, prescribing, and monitoring as needed.
- Assist the program with medication monitoring, including spot-checking medication documentation to help ensure compliance. Communicate with programs, external prescribers, and pharmacies/medication suppliers.
- Order and follow up on laboratory tests and procedures related to practice.
- Collaborate with the clinical team to prepare and update client treatment plans. Communicate with internal and external treatment providers to coordinate treatment and service delivery. Participate in weekly team meetings and case conferences as needed. Be available for consultation regarding cases.
- Document services provided in medical/case records, including requirements to document in both AWARDS and CARES electronic case record systems.
- Participate in supervision with the Program Director.
- Assist with crisis intervention, as needed, which may include clients experiencing recent decompensation and/or acute crisis situations as they arise during hours of operation.
- Maintain training in First Aid, crisis de-escalation, and overdose prevention.
- During hours of operation, be flexible and available to interact with the client population throughout the facility.
- Board-certified Psychiatric Nurse Practitioner with relevant experience or a strong interest in working in this rewarding setting.

- Ability to work collaboratively within a multidisciplinary treatment team.
- Excellent documentation, communication, and organizational skills.
- Experience working with individuals experiencing chronic homelessness, serious mental illness, and/or substance use disorders preferred.
- Strong skills in psychiatric assessment, diagnosis, and psychopharmacological management.
- Bilingual Spanish proficiency is highly desirable.
