About the Role

New York City Health and Hospitals Corporation’s Outposted Therapeutic Housing Units Program (OTxHU) provides comprehensive health care to incarcerated patients in NYC jails. CHS, the direct provider of health care in the jails, delivers medical, nursing, mental health, pharmacy, substance‑use treatment, social work, dental and vision care, discharge planning, and reentry support. The OTxHU bridges the continuum between jail care and inpatient hospitalization, with admissions and discharges based on clinical necessity.

The first unit will open at NYC Health + Hospitals/Bellevue in Manhattan, with a projected completion date as early as the end of 2024.

Responsibilities
  • Diagnose and treat acute and chronic illnesses; submit prioritized consults when indicated.
  • Complete comprehensive histories and physicals for new admissions, including documentation of problems, diagnoses, orders, and appropriate medication orders.
  • Evaluate patients requesting sick call; schedule follow‑ups; update medication orders and problem lists at all visits.
  • Implement patient‑care plans using protocols approved by the Medical Director and current standard of care.
  • Order and interpret radiology exams, lab tests, and EKGs in consultation with supervisors as needed.
  • Review specialty consults and hospital returns to ensure standard of care and implement consultant recommendations.
  • Perform chart reviews and summaries for patient transfers; update problem lists and medication orders.
  • Generate special needs referrals as needed (e.g., disabilities, dietary restrictions, heat sensitivity).
  • Educate patients about conditions and treatments; counsel on risks and benefits; witness and document refusals of care.
  • Ensure all progress notes and orders are signed before shift end.
  • Respond to emergencies promptly and professionally.
  • Notify the staff MD and Urgicare about 3‑hour runs and EMS activation.
  • Complete special housing rounds when assigned.
  • Maintain knowledge of quality‑of‑care and population‑health indicators; take action to meet or exceed standards.
  • Maintain clinical competency by participating in CME, CHS training, and in‑service requirements.
  • Adhere to scheduled hours, including punctuality and timely absence notification.
  • Follow CHS policies and procedures.
  • Complete tasks delegated by the Site Medical Director or supervising clinical team member.
  • Maintain documentation of all required credentials.
  • Present a professional attitude and appearance.
  • Adhere to Occupational Health Services requirements.
Minimum Qualifications
  • Valid limited permit to practice as a Physician Assistant issued by the New York State Education Department (NYSED); or
  • Valid license and current registration to practice as a Physician Assistant issued by NYSED.
  • Valid Life Support certification (e.g., BLS or ACLS) within sixty (60) calendar days of appointment.
Preferred Experience and Skills
  • Experience with patients in skilled nursing or residential settings.
  • Experience with patients who have serious mental illness.
  • Experience with patients who have substance‑use diagnoses; knowledge of harm‑reduction approaches and opioid use disorder medications.
  • Experience leading quality‑improvement initiatives.
  • Understanding of trauma‑informed care.
  • Proficient in patient‑centered shared decision‑making.
  • Skilled communication of risks and benefits; assessment of informed decision capacity.
  • Active BLS and ACLS certifications.
  • Infection control certification.
  • Excellent interpersonal communication skills and ability to work collaboratively in multidisciplinary teams, including with NYC DOC staff.
  • Flexible disposition.
