Job Duties
Complete progress notes and referrals written by rehabilitation staff, ensuring compliance with recordkeeping requirements and standards.
- Screen, evaluate, and approve all referrals.
- Match and assign patients to appropriate daily and inpatient programs.
- Monitor the tracking system and the delivery of rehabilitation programming.
- Assure patient records and rehabilitation documentation are maintained as outlined.
- Coordinate treatment planning with the rehabilitation department and treatment team.
- Complete a comprehensive evaluation of rehabilitation and social needs and develop patient goals and programming.
- Review policies, procedures, and available rehabilitation programs with staff.
- Coordinate development of goals and programs with subordinate rehabilitation staff and treatment teams.
- Participate with the Treatment Team Leader in policy meetings, long‑term program planning, and review meetings to improve programs using evidence‑based practices.
- Identify patient needs for referrals to other specialists, advise patients, and communicate with referral sources as appropriate.
- Coordinate services, interact with significant others, place patients, and follow up on progress while solving problems.
- Determine changes due to planned programs by comparing actual vs. desired results.
- Act as the rehabilitation liaison for services provided by the department.
- Arrange psychosocial, educational, and vocational services as needed.
- Provide inpatient treatment teams with objective information concerning patient performance in residential, social, and daily living skills.
- Interpret, report, and document all ongoing patient progress.
- Assist patients with interpreting daily activities and their impact on rehabilitation.
- Complete incident reporting and conduct investigations.
- Document findings and recommendations to ensure timely information for treatment team members.
- Coordinate person‑centered service plans and monitor client progress, consulting with the treatment team to modify plans as needed.
- Participate in treatment team meetings, making recommendations and implementation suggestions to achieve desired outcomes.
- Collaborate with individuals, families, advocates, care managers, and service providers to determine service delivery strategies, attend Comprehensive Functional Assessment meetings, and establish habilitation objectives.
- Assess individuals’ skill levels relative to valued outcomes and identify effective teaching methods.
- Communicate with treatment team members and gather information to plan, modify, and meet needs.
- Implement safeguards and protective oversight to protect health and welfare while participating in programs.
- Monitor, record, and measure progress toward goals, modifying plans to maintain progress and prevent regression.
- Attend annual/semiannual Life Plan meetings.
- Assist with development and review of Staff Action Plans.
- Ensure completion and quality review of Monthly Summary Notes per policy.
- Review EHR/DSR and peer review daily.
- Attend daily conference rounds and individualized meetings.
- Supervise, coordinate, and direct activities of DSA’s, Hab Specs and Recreation.
- Monitor, review, and complete TABS.
- Oversee staff assignments, evaluate performance, monitor time and attendance.
- Oversee physical plant maintenance and housekeeping for health and safety.
- Communicate program objectives, techniques, and ICF policy to staff.
- Maintain habilitation records and required documentation in line with ICF regulations.
- Monitor and enroll staff in in‑person trainings; update SLMS trainings.
- Complete quarterly assessments.
- Gatekeeper: monitor and verify all documentation for accuracy and currency in EHR.
- Perform other duties as directed by the TTL and Administration.
Promotion:
One year of permanent service as a Rehabilitation Counselor 1 and a qualifying bachelor’s or higher degree; applicant must be reachable on the current Civil Service Eligible List for this title.
