Overview

Flagstar Nursing is seeking Certified Occupational Therapist Assistants (COTA) for Skilled Nursing Facilities. Now staffing COTAs for Rehab Departments throughout NYC and Long Island. Flagstar Rehab COTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in daily life. Flagstar Rehab offers opportunities for Direct Employment with our agency or to be your own boss as an Independent Contractor.

Hourly Pay Rate: $33-$34 Hourly (W-2), $37-$38 Hourly (1099)

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.

• Current NYS OTA License and Registration
• EMR Competence
• State Required Medical Documentation for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
