• Manage messages and handle telephone encounters to support efficient patient care
• Assist the call center nurse in providing necessary information for scheduled visits
• Support post-discharge appointments, referrals, and DME orders
• Collect discharge records, medical histories, and treatment plans as required
• Perform other job-related duties as needed to ensure smooth operations

• High School Diploma required
• Medical Assistant Certificate or completion of an accredited Medical Assistant program
• Bilingual in Spanish and English
• Minimum of 2 years of established Medical Assistant experience
• Proficiency with computers and strong communication skills

• Previous experience working in a community-based health center or similar setting
• Flexible work schedule and ability to multitask
• Familiarity with telehealth services and patient care protocols
