Job SummaryThe Day Habilitation Specialist is responsible for assisting consumers with their daily programmatic responsibilities and providing the consumers with training that supports independent living skills.
Duties and Responsibilities- Receives and completes assignments as directed by the Coordinator of Day Habilitation Services.
- Provides direct supervision of assigned caseload.
- Assists the program participants, care manager, and advocate in creating an individualized Life Plan.
- Attends meetings, completes daily billing requirements, weekly service notes, behavior data and any other documentation required.
- Ensure all work areas of site is cleaned and disinfected daily.
- Provides the program participants with the necessary training, support and opportunities to achieve outcomes such as:
- Increase sense of self-esteem
- Increase relationships with others
- Increase choice making skills
- Development of new skills
- Improvement of existing skills
- Community participation and integration
- Productive use of time
- Plan and implement person centered activities throughout the day.
- Increase self-advocacy skills
- Adheres to OPWDD and IGHL policy, including Safety and Code of Conduct; complete agency forms as required: time off requests, tuition reimbursement, physical complaint, non-reportable and incident reports; utilizes time system correctly.
- Enforces and adheres to all safety policies and procedures.
- All other duties assigned.
- Responsible for transporting consumers to and from programs and/or daily activities.
- High School Diploma/GED
- One year experience working with the disabled population preferred
- Valid NYS driver’s license
- Complete and maintain all certifications
- Experience or interest in instructing the developmentally disabled during real life experiences
- Must be able to lift 50 pounds
