Position Summary

Responsible for assisting with the overall administration of the CHN Care Coordination program. This includes clinical supervision of the Care Coordinators and Interns, coordination of client services, staff recruitment and hiring, staff training, community and case finding, weekly, monthly and quarterly reporting of program information to the Program Director, implementation of the program policies and procedures.

Duties And Responsibilities
  • Works with the Director to provide strategic, substantive, and administrative leadership in implementation of policies and procedure for CHN’s Care Coordination Program.
  • Systematically develops, implements, and monitors multiple organizational and operational objectives
  • Effectively communicates with and guides the Care coordinators to accomplish the program s goals and objectives and facilitates clinical insight on programmatic and client related issues.
  • Administers and provides personnel services in the recruitment, selection and orientation of new employees
  • Performs Quality Assurance and Quality Improvement reviews.
Certificate/License Required
  • Masters degree in Social Work preferred or Masters level with commensurate clinical training and experience.
