• Coordinate and link patients with additional services
• Provide crisis intervention, stabilization, and brief counseling as needed
• Provide proactive behavioral health case management to clinic patients
• Employed position
• Full-time Monday through Friday work schedule
• Fluency in an Asian language or Spanish is preferred but not required
• Ability to work with limited-English-proficient populations
• Master's degree in related fields
