We are building a talent pool of strong clinical professionals for Freedom Care New York. The positions include:

• RN Nurse Educator – Provide education, support, and resources to caregivers in an office setting to help them become certified Health Home Aides.
• RN Field Nurse Assessor – Conduct patient assessments to determine care needs and develop personalized care plans.

• Multitasking – Manage multiple responsibilities and priorities while guiding patients and caregivers.
• Clinical experience – A valid and active NYS nursing license, strong knowledge of geriatric care and experience conducting assessments.

Base pay range: $100,000—$110,000 USD per year for the first year. Compensation decisions are based on skill set, experience, education, location, and licensure.
