Role Description
The Director of Chronic Care Management & Clinical Operations is a full-time on-site position based in New York, NY. The role involves overseeing chronic care management programs, ensuring high-quality patient care, and optimizing clinical operations. Responsibilities include leading case management initiatives, developing and implementing care plans, training healthcare staff, and collaborating with multidisciplinary teams to enhance clinical outcomes. The Director will drive continuous improvement initiatives and ensure compliance with healthcare standards while providing leadership and support to clinical teams.
Qualifications
- Strong background in Nursing and Case Management
- Experience with Critical Care Medicine and Medicine practices
- Proven ability to develop and deliver effective staff Training programs
- Excellent leadership, communication, and organizational skills
- Ability to manage multiple priorities and optimize clinical operations
- Bachelor's or advanced degree in healthcare-related fields such as nursing or medicine
- Experience in chronic care management or clinical operations is highly preferred
- Commitment to patient-centered care and continuous process improvement
Detailed Responsibilities
1. Strategic Program Expansion & Implementation
- Specialty Integration:Design and launch the clinical workflows for Behavioral Health Integration (BHI) andWound Careprograms, including the selection of screening tools (e.g., PHQ-9, GAD-7) and tissue viability protocols.
- SOP Development:Author and maintain
Standard Operating Procedures (SOPs) that standardize how the 50+ staff handle specialized patient escalations. - Technology Optimization:Partner with IT/Product teams to customize theEHR or CCM platform for better tracking of wound healing progress and mental health status.
2. Operational Leadership & Scale
- Manager of Managers:Provide leadership to 5 Team Leads, conducting weekly "Lead-Level" audits to ensure their respective sub-teams of 10 are meeting outreach andbilling minute targets.
- Capacity Planning:Manage staffing ratios and recruitment strategies to ensure the team can scale as patient enrollment increases.
- Performance Metrics:Monitor and report on high-level KPIs includingNet Promoter Score (NPS),HEDIS quality scores, and reduction inEmergency Department (ED) utilization.
3. Compliance & Clinical Quality
- Audit Oversight:Lead internal audits to ensure 100% of care plans are "patient-centered" and updated at least annually perCMS guidelines.
- Risk Management:Act as the final clinical escalation point for complex patients requiring multi-disciplinary intervention between primary care, mental health, and wound specialists.
Requirements
- Education:BSN required;
Master’s in Healthcare Administration (MHA) or Nursing (MSN) highly preferred. - Experience:7+ years in clinical care management with at least 3 years managing "managers" in a high-volume ambulatory setting.
- Expertise:Deep knowledge ofCMS Fee-for-Service andValue-Based Caremodels.
