Position OverviewThe role will involve facilitating transition from inpatient detox and/or rehabilitation to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through group and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure they become linked and engaged with identified community providers and that their support network is sufficient to meet their needs.
The role requires on‑call coverage and a willingness to work flexible hours.
- Engagement beginning during inpatient detox or rehab stay, involvement in discharge planning, and a needs assessment of community transition supports essential to stabilizing the individual
- Individual or group support sessions with emphasis on relapse prevention and preparing for independent community living and the promotion of optimum mental and emotional health
- Helping individuals deal with issues associated with addictions and substance use, family and social relationships, stress and symptom management, activities of daily living, medication management, and housing readiness; work with recipients and their housing providers to resolve clinical issues impacting the ability to obtain, manage, and retain supportive housing
- Assist recipients in applying and navigating SSI/SSD, Medicaid, and other necessary benefit services
- Foster relationships and advocacy with community providers to ensure recipients are connected with appropriate services as they transition back into the community and to expedite and solidify placement in community housing
- Community navigation including accompanying to first behavioral health and medical appointments, travel training, re‑engagement in community care, referral to services with ability to identify and address potential barriers to obtaining and maintaining such services
- Establish collaborative working relationships with inpatient treatment teams and other partners and plan with them for appropriate discharges for recipients
- Provide intensive emotional and practical support to recipients as they transition back into their communities
- Provide out‑of‑hours crisis intervention services to recipients and their support network, including respite referrals and other diversion and stabilization services
- Assess housing supports and housing assistance and placement for those who are homeless; services may include completing HRA 2010e, applying for housing, prepping for interviews, follow‑up with housing providers, and assistance with moving in (day of move) with obtaining housing supplies and learning the neighborhood
- Attend and participate in team meetings and supervisory sessions
- Develop short‑term person‑centered treatment plans to assist recipients toward achieving their goals related and unrelated to their recovery
- Monitor and record recipients’ progress with respect to treatment goals
- Assist and instruct individuals in attending to personal hygiene, grooming and laundry, nutrition and meal preparation, budgeting, and other daily living and socialization tasks and skills needed to live, work, and socialize successfully in the community environments of their choice and also in maintaining a safe living environment
- Comply with all required in‑service training and staff development
- Perform other related duties as assigned
- Bachelor’s degree in psychology, social work, sociology, or related field
- 2 years’ experience in case management from a social service agency or serving mental health and/or substance‑using populations
- Knowledge of substance use, mental illness, treatment options, and the needs of an individual integrating back into the community
- Excellent written, verbal and computational skills to present and document records in accordance with program standards
- Master’s degree
